Note: The words that contain all capital letters in them are supposed to be edited as you wish by yourself.

Basic HTML Page structure and setup:

Put this between the tags near the HTML on the top of the page and edit it as you wish:

Next, add the textarea tags (like the head tags, but with "textarea" in them and text between them) wherever you would like these text boxes, like I am using for the HTML codes in this page.
